When I keep pushing, my elbows spread out to the side, so I need to hold it in a way that prevents that, right?

The target area changes when you do it before the assembly and when it is fixed. As you mentioned, if you externally rotate and spread your elbows, pushing in a straight line will target the outer part of the lateral head of the triceps. If you slightly internally rotate and keep them from spreading while pushing diagonally, it will target the long head.
